Does Insurance Cover IOP?
For most people with a PPO or HMO plan, yes, and finding out for sure takes minutes, not weeks. Here is how coverage actually works, in plain language.
Written by the Glendora Recovery Center care team. Last updated July 2026.
The short answer
Most major PPO and HMO plans cover intensive outpatient treatment, for mental health and for substance use. Federal parity law requires most plans to cover mental health and substance use care comparably to medical care, which is why IOP sits inside mainstream coverage rather than outside it.
The honest caveat: "covered" is not the same as "free," and every plan draws its own lines. That is exactly why verification exists, and why we do it for you before you commit to anything.
We are in-network with most major PPO and HMO plans. We do not accept Medi-Cal at this time.
What actually affects your cost
Four pieces of your plan decide what IOP costs you out of pocket:
- Your deductible: what you pay before the plan starts sharing costs, and how much of it you have already met this year
- Your copay or coinsurance: the share you pay per session or per service once the deductible is met
- Your out-of-pocket maximum: the annual ceiling after which the plan pays in full
- Network status: in-network care almost always costs less, which is why the first question worth asking is whether a program is in-network with your plan

Who are you here for?
A few quick taps, completely confidential. Answer right here on the page.
How free verification works
You give us your insurance details, either through the secure form on our verify insurance page or over the phone at (626) 594-0881. Our admissions team contacts your insurer, confirms your IOP benefits, and walks you through what your plan covers and what, if anything, you would owe, usually within minutes and often the same day.
It is free, it is confidential, and it creates no obligation. Checking your benefits does not commit you to treatment, does not notify your employer, and does not affect your coverage. It just replaces dread with a number.
If the math still feels impossible
Do not let the fear of cost end the conversation before it starts. Plans differ, situations differ, and admissions has seen nearly all of them. If you need to apply for coverage, or you are between plans, call anyway: the team can point you in the right direction either way, and the call costs nothing.

Frequently Asked Questions
Straight answers to the questions we hear most. Anything else? Call us, day or night.
Does insurance cover IOP for mental health?
Most major PPO and HMO plans do. Federal parity law requires most plans to cover mental health care comparably to medical care. Your exact cost depends on your deductible, copay or coinsurance, out-of-pocket maximum, and network status, which is what free verification confirms.
How long does insurance verification take?
Usually minutes, and often the same day. Submit the secure form on our verify insurance page or call (626) 594-0881, and admissions confirms your benefits directly with your insurer.
Does checking my benefits obligate me to anything?
No. Verification is free, confidential, and creates no obligation. It does not notify your employer and does not affect your coverage.
Do you accept Medi-Cal?
Not at this time. We are in-network with most major PPO and HMO plans. If you have Medi-Cal, call anyway and we will do our best to point you toward options.
Will my employer find out I am in treatment?
Not from us. Your treatment records are protected by federal confidentiality law, 42 CFR Part 2, and HIPAA. Verifying benefits and receiving treatment are between you, your insurer, and your care team.
You do not have to do this alone.
